Release and Waiver of Liability
In consideration of my participation in all activities for the Anchor Point Church camp out, I agreed to release and hold harmless the Anchor Point Church, its respective officers, agents, representatives, successors, assigns and sponsors and all individuals associated with the event, from any and all liability (direct or indirect), for any injury or harm of any kind to me as a result of my participation in event activities or which might be incurred while I am in transit to or from the event.
I also give my permission to the chaperones to seek any necessary medical attention for me during this event. This notification will also include the right to sign for, but not limited to, any emergency service transport, hospitalization, or emergency services or surgery. I further agree to be responsible for the cost of such treatment, including but not limited to, costs associated with transportation to the treatment facility.
To express my understanding of the above release and waiver of liability for myself and all campers in my party, I sign here.
