In consideration of my participation in all activities for the following event, Junior-SENIOR Banquet, I agree to release and hold harmless AnchorPoint Church, its respective officers, agents, representatives, successors, assigns and sponsors and all individuals associated with the event, from any and all liability (direct or indirect). I give my permission to the chaperones to seek any necessary medical attention for me during this event. This notification will also include the right to sign for, but not limited to, any emergency service transport, hospitalization, or emergency services or surgery. I further agree to be responsible for the costs of any such treatment. I further agree to abide by the dress code, code of conduct and guidelines of AnchorPoint Church and be attentive to chaperones on site. Dress Code: Boys: Suit/Tie or Tuxedo Girls: Dresses, no low cut (front or back), not clingy or sheer. By checking this box I agree to the terms above.
